1. 1-Hour Initial Consultation
Conducted by Kylie Toynton, Certified Practising Speech Pathologist.
A detailed discussion to:
Understand the participant’s needs, goals, and interests.
Link program objectives to the participant’s NDIS goals, such as improving communication, social skills, independence, or daily living activities.
Develop a personalised therapy plan based on functional outcomes.
2. Personalised Therapy Program (5 Hours of Equivalent Therapy Support)
Delivered as a mix of video and written materials tailored to the participant’s specific developmental goals.
Video Materials:
Step-by-step demonstrations of strategies and activities to ensure parents and caregivers feel confident implementing the program.
Written Materials:
Detailed instructions and guidance on integrating therapeutic activities into everyday routines.
Incorporates evidence-based approaches such as:
Therapy Outcome Measures (TOMs) are integrated into the program to monitor and evaluate progress across key domains, including activity, participation, and well-being.
3. Resource Kit
A curated set of tools to support the implementation of the personalised therapy program.
Designed to motivate and engage the participant while focusing on measurable therapeutic outcomes.
Emphasis on resources that enhance the caregiver’s confidence in delivering the activities effectively.
4. Follow-Up Support
Access to a follow-up program with the Speech Pathologist for ongoing support.
Includes:
Adaptations or refinements to the program based on progress.
Email or phone support to troubleshoot challenges.
Suggestions for further enhancing the participant’s engagement and outcomes.
5. Long-Term Goal Alignment
The program is designed to set a foundation for sustained progress and can be extended or adapted as part of ongoing therapy support.
